Services The Elder Service Plan coordinates all of the services offered. Each Elder Service Plan participant and their caregivers become part of a care team. The PACE team includes social workers, Board certified geriatricians, nurse practitioners, nurses, occupational and physical therapists and nutritionists. Participants will undergo an evaluation by the team to determine what kind of services he/she require. All services are provided by ESPNS providers and ESPNS contracted providers and facilities. ESPNS participants who access non-emergency services without ESPNS authorization may be personally liable for the resulting costs of these unauthorized services SOME OF OUR SERVICES INCLUDE:
