The Elder Service Plan of the North Shore (ESPNS) is a non-profit organization dedicated to maintaining the health and social life of older adults so that they may continue living independently in the community.

-Clinical Positions-

 

Social Worker  
Location: Beverly

This is a generalist practice full-time position that serves as a member of the PACE Interdisciplinary Team. The PACE Social Worker provides traditional case management services within an all-inclusive medical model of care. The Social Worker conducts psychosocial assessments, participates in the development of individualized care plans, and provides education and advocacy for participants and their caregiver/families. This professional also serves as the liaison between PACE and externally contracted partners including hospitals, nursing facilities, and home care agencies. The position requires a Masters Degree in Social Work with licensure preferred at the LCSW or LICSW level, and at least one year of experience working with a geriatric population. Nurse Practitioner
Location: Buffum
The Elder Service Plan of the North Shore, a sponsor of the PACE Program, is seeking a full-time Nurse Practitioner interested in an outpatient geriatric/adult medicine position in the Greater Lynn area. The Nurse Practitioner works in tandem with the primary care physician and serves as a member of the PACE Interdisciplinary Team. PACE is a nationally recognized all-inclusive medical model of care for frail elders, dedicated to a mission of keeping its elders living at home in the community for as long as they may do so safely. This individual must be eligible for Board Certification as a Nurse Practitioner in the Commonwealth of Massachusetts with MA and DEA Prescription Privileges, and at least one year of experience working with a geriatric population.

PCA
Location: Beverly/Gloucester
Dependable CNA/HHA certified individual under the direction of a registered nurse needed to assess and assist with the personal care needs of participants as needed. Participate as a member of a multidisciplinary team. Geriatric care preferred, current certification required. Proficiency with Microsoft Office a plus.

PCA
2 Full Time dependable PCA, CNA/HHA certified, to work under the direction of a registered nurse. Assess and assist with the personal care needs of participants as needed. Psychiatric Advanced Practice Nurse
A full or half time Psychiatric NP or CNS to function as a member of a multidisciplinary team that provides comprehensive services to maintain older adults with functional deficits in the community. Working in close collaboration with a geriatric psychiatrist, and other behavioral health professionals, this position involves evaluation and treatment of older adults, including medication management and psychotherapy, and consultation to other professionals, including residential care providers. Patient population includes individuals with new onset mental health disorders and individuals with long standing conditions.

Minimum qualifications: ANCC certification and license to practice in Commonwealth of MA as a Registered Nurse/Prescribing Clinician (RN/PC). 3-5 years in advanced practice role with prescriptive authority. Demonstrated competence with evaluation and treatment of older adults, including psychopharmacology, crisis evaluations and consultation to caregivers.

Registered Nurse
Geriatric and community experience required. Duties include on-site and in-home nursing assessment, care plan development, and supervision of Personal Care Attendants. Participates as a member of an interdisciplinary team. The position requires “ On–Call” responsibilities. Candidates must demonstrate the knowledge and skills necessary to assess plan of care and provide service to an elderly population. At least two years experience with the elderly population in a community based program required.

Kitchen Aide
Location: Gloucester
Serves as a member of PACE Interdisciplinary Team. This individual reports to the Site Coordinator and is responsible for managing all aspects of the food service operation at the site. This individual must demonstrate profi ciency in general food service and be committed to the ideals of caring for frail elders in a respectful and dignified manner, with the goal of assisting the elders to remain living in the community for as long as they can do so safely. High School diploma or equivalent with ServSafe Certification required. Minimum 1 year of experience in food service or equivalent preferred.

Activity Assistant
Location: Beverly
Part time activity assistant to work with a multidisciplinary team under the direction of an Activity Director, assisting with individual and group activities. Must be energetic, willing to learn, and have a strong desire to work with the elderly in a fast paced environment. High School diploma or GED required with at least two years of work experience. Experience working with adults with dementia a plus.

Medical Secretary
Site: Berverly
Working under the direction of the Site Coordinator and Physician/Nurse Practitioner team, the Medical Secretary is responsible for the scheduling of all external medical appointments for the PACE participants, manages participant related transportation needs through contracted vendors, maintains all of the participants medical records and tracks and updates relevant data and reports.
The position requires a High School Diploma or equivalent and candidates must have 2 years of experience as a secretary preferably as a Medical Secretary, and proficiency with Microsoft Office products. Certificate in Medical Terminology, and Electronic Medical Records experience also preferred. Bilingual Spanish is a plus. The position also requires a highly organized individual with strong verbal skills who functions well in a fast paced health care setting.

Medical Records Coder
Twenty-five (25) hours per week with the potential to move to a full-time (40) forty hour per week position. The Medical Records Coder is responsible for preparing and submitting Medicare bills for medical service payments in accordance with required procedures.  This individual must possess a comprehensive knowledge of (CPT, HCPCS, ICD-9-CM) diagnosis and surgical codes, HIPAA, subscriber and certificate language, reimbursement and medical policy to operationalize payment policy coding options and edits.  The Coder is expected to develop and maintain system editing criteria that supports savings to the organization across all service lines, and to generate new ideas for savings and validate clinical appropriateness of such edits with organizational medical experts.
The position requires a Bachelor’s Degree in business, medical profession or related field, and certification as a CPC Certified Professional Coder.  Three to Five years of coding experience required.  The ideal candidate possesses excellent written and verbal communication skills, research skills, and the ability to work as a part of a team and independently.  Competitive salary and generous benefits package included.  EEO Employer.
