Eligibility

Participants must meet certain Federal and State eligibility requirements. These are:

  • Must be 55 years old or older
  • Must live within the ESPNS service area which includes the following towns: Beverly, Danvers, Essex, Gloucester, Hamilton, Ipswich, Lynn, Lynnfield, Magnolia, Manchester, Marblehead, Middleton, Nahant, Peabody, Rockport, Salem, Saugus, S. Hamilton, Swampscott, Topsfield, Wakefield and Wenham.
  • Must be certified as eligible for nursing home care (must require help with two "activities of daily living" and at least one "skilled nursing" need).
  • Must have the ability to continue living at home and must be able to do so safely with ESPNS medical and support services

Care is coordinated by our interdisciplinary team, so information and services are less fragmented. Each participant has an individual plan of care which may include services such as Home Care, Medications, Primary Medical Care, Adult Day Health Center and Transportation.

                                

PACE Adult Day Health Centers are open Monday through Friday, 8 am to 4 pm.  Although the day centers are open during normal business hours, a PACE clinician is on call 7 days a week; 24 hours a day, 365 days a year.
